Health Wizz is partnering with Cape Fear Valley Hospital in Fayetteville, N.C., to pilot its mobile application that is designed to help users aggregate, organize and share their medical records.
The app is free and available at Apple's App Store and Android's Play Store. It allows patients to download and standardize their health records on their mobile phone as well as share their data with multiple parties, such as pharmaceutical companies that are conducting clinical trials. Health Wizz had now added enhancements to the app, including a social connectivity feature, enabling users to create their own health-related challenges.

Thirty patients from Cape Fear Valley Hospital will enroll in a chronic disease management program in the next few weeks. They will use the Health Wizz platform for 60 days. The pilot will focus on Medicare beneficiaries with congestive heart failure, with the goal of reducing hospital readmission within 30 days of discharge and improving outpatient care.
"The Health Wizz platform allows providers to conduct outreach and consistent follow-up, monitor and manage patients more effectively in their homes, while cultivating a deeper understanding of chronic diseases," said Nitin Desai, MD, CMO of Health Wizz, who is overseeing the pilot. "It will allow patients to collect data, deliver more accurate and timely information to the point of care, and help clinicians understand how to better treat their patients."
