Atlanta-based Piedmont Healthcare is launching a new continuous care program at two of its medical facilities with the aim of improving collection of patient information from remote care devices.
The program uses remote patient monitoring software from Telemetrix combined with coaching provided by remote care management company Remote Care Partners, according to a May 11 press release from Telemetrix.
The program is also integrated into the health system's provider workflow.
Piedmont Healthcare says it plans to roll out the program at additional facilities.
