Madison, Wis.-based insurance company Dean Health is reporting a breach of protected health information that may affect 960 patients, after documents mailed to a bank were lost, according to the Wisconsin State Journal.
The documents included member names, health plan member identification numbers and procedure codes, but did not contain Social Security numbers, addresses, financial information or phone numbers.
Dean Health said it learned of the breach once an envelope containing a small portion of the documents initially sent out was returned. The package was sent to a bank that processes payments for a hospital, but the bank says it never received the documents, according to WSJ.
The insurer is in the process of notifying the members whose information was lost.
