"[O]ut of an abundance of caution," the University of Oklahoma has mailed notification letters to 9,300 urology patients informing them that a laptop containing health data was stolen from a former OU urologist.
The theft occurred mid-July, according to a statement from the university, and although the physician immediately reported the theft to the police, the university determined the patients whose information may have been compromised had not been informed.
The university and the physician are unsure what information was contained on the laptop, but said spreadsheets on the computer may have contained limited information from pediatric urology procedures conducted from 1996 to 2009, such as patient name, diagnosis and medical treatment.
Although Social Security numbers, addresses and financial information were not included, OU is offering complementary one-year subscriptions to credit-monitoring services for affected patients.
