The employee responses to the phishing emails contained affected patients' names, addresses, dates of birth, telephone numbers, treating physicians, diagnoses, treatments received, medical record numbers, medical service codes and health insurance information. Some patients' Social Security numbers were also included in the emails.
FMG began notifying affected patients of the data breach on March 28 and has no evidence the compromised patient information has been fraudulently used.
