- HOPE Family Health in Westmoreland, Tenn., announced a data breach involving a laptop stolen from an employee's home. About 8,000 current and former patients' information was compromised.
- An unencrypted laptop containing the personal and health information of 3,541 patients was stolen from the car of a University of California San Francisco Medical Center employee.
- A phishing scam targeting St. Louis University employees led to the disclosure of the personal health information of approximately 3,000 people.
- During a routine internal audit Aug. 8, officials at Gastonia, N.C.-based CaroMont Health discovered 1,310 patients' health information had been transmitted via unsecured email.
- Des Moines, Iowa-based UnityPoint Health announced a data breach affecting up to 1,800 patients.
- St. Mary's Janesville (Wis.) Hospital began alerting 629 patients who visited the hospital's emergency department between Jan. 1 and Aug. 26 of a possible data breach.
- Santa Clara Valley Health & Hospital System, based in San Jose, Calif., sent letters to patients affected by a recent data breach.
- Holy Cross Hospital in Ft. Lauderdale, Fla., sent letters to 9,900 former patients informing them of a possible data breach.
