1. Buffalo, N.Y.-based Dent Neurologic Institute announced that an administrative clerk accidentally emailed the personal information of 10,000 patients to approximately 200 patients.
2. Presbyterian Anesthesia Associates in Charlotte, N.C., disclosed that a hacker gained access to the personal information of 9,988 people by breaking through a security loophole on the practice's website, according to a Charlotte Observer report.
