$20M investment helps former Livongo exec launch rural hybrid care startup

Improving rural healthcare disparities will be the focus of a startup launched by Jennifer Schneider, MD, who helped build Livongo, and other former Livongo team members.

Homeward, which launched March 7 with a $20 million investment, will be the first comprehensive provider to take on full risk in rural markets, according to the press release. The company will initially partner with regional Medicare Advantage plans and will provide the following:

  • A hybrid model of technology and services that will connect rural patients to primary care and specialty services.

  • A virtual and in-person mobile care unit with in-home remote monitoring.

  • Cellular-based monitoring devices and virtual services to connect rural patients to care, no matter their location.

The startup hopes to combat the issue of healthcare access that 60 million Americans in rural communities face.
