Trillium Community Health Plan partnered with technology company Unite Us to enhance the health plan's ability to collaborate between providers and community-based organizations to provide healthcare services for Oregon Health Plan members.
The Connect Oregon network, which uses Unite Us' technology platform, will be used by Trillium's care managers and member services staff to coordinate care between health and social service providers, according to an Aug. 19 news release.
Trillium CEO Sarah Brewer said the company is committed to ensuring members have access to equitable, culturally sensitive care.
"Through the Connect Oregon network, we can facilitate engagement with a variety of services for our members while actively collaborating with our provider and community partners to assist if more care is needed along the way," she said in the release.
Under Trillium's health equity strategy, the plan will analyze metrics to track health outcomes and inform future partnerships and initiatives designed to invest where there is a community need, the release said.
