Paso del Norte Health Information Exchange selected Berkeley, Calif.-based health tech company Innovaccer to integrate patient information across six practice sites, the company announced Nov. 7.
The El Paso, Texas-based health information exchange aims to integrate information across multiple practice sites on three different EMRs to provide clinicians with access to a patient's longitudinal health record.
The HIE will leverage Innovaccer's Care Intelligence System to standardize data from across the care continuum. With the platform, providers will be able to analyze performance metrics related to chronic care, preventive screening, pediatric health and medication management to identify actionable insights.
