Two employee email accounts associated with Plano, Texas-based Live Oak Surgery Center were compromised by an unauthorized third party, which in turn potentially caused the information of 5,264 patients to be viewed or taken.
Live Oak said between Aug. 10 and Sept. 27, two employee email accounts were found to have been accessed by an unauthorized user, according to a Jan. 3 breach notification from Live Oak.
An investigation that concluded Nov. 17 revealed the following patient information was stored within one of the email accounts: names, financial account information, dates of birth, payment card information, medical information, health insurance information, passport numbers, Social Security numbers, driver's license numbers, and usernames with passwords.
The surgery center said it is not aware that any of the information within the emails has been misused.
Live Oak is currently mailing letters to all affected individuals and has implemented additional security measures to further protect information.
