CMS is accepting applications for the State Innovation Models initiative, which is a $275 million competitive funding opportunity for states to design and test multi-payor payment and delivery models.
CMS' Innovation Center created the State Innovation Models initiative for states that are prepared for or committed to planning, designing, testing and supporting evaluation of new payment and service delivery models in the context of larger health system transformation. The CMS Innovation Center was created by the Affordable Care Act to test innovative payment and service delivery models that have the potential to reduce program expenditures while preserving or enhancing the quality of care. The goal of the State Innovation Models initiative is to create multi-payor models with a broad mission to raise community health status and reduce long-term health risks for beneficiaries of Medicare, Medicaid and the Children's Health Insurance Program.
States may apply to the initiative through the Grants.gov website. To do so, states will need to include the following in the application:
• Employer identification number, otherwise known as a taxpayer identification number.
• Dun and Bradstreet Data Universal Numbering System number.
• Registration in the system for award management database.
• Authorized organizational representative.
