How different health systems have partnered with payers to improve community health disparities

As the payer-provider relationship evolves, there is a growing consensus that the two parties need to increase collaboration to be profitable.

The relationship no longer feels like an option but a mandate. A few innovative providers in Chicago have worked together to not only bolster their bottom line but to improve the health disparities of the communities they serve. As these two groups become increasingly intertwined, hospitals are leveraging these relationship to create mutually beneficial outcomes for all parties — payer, providers and patients. These partnerships are going beyond claims and addressing how population health starts with preventive care.

For example, RUSH created the Health Legacy Program for African American women to reduce the incidence of diabetes through weekly health education, nutrition counseling, exercise and support group sessions. Lurie Children's Hospital is developing tools that allow clinicians to provide interventions and track outcomes based on a disparity assessment. AMITA Health is partnering with public libraries to provide social workers on site.
