Hearst Health has named three finalists for the Hearst Health Prize, a $100,000 award given in recognition of outstanding achievement in managing or improving health.
Hearst Health is a unit of the media and information company Hearst, and the Hearst Health Prize is a collaboration with the Jefferson College of Population Health of Thomas Jefferson University in Philadelphia. Hearst Health announced the launch of this inaugural prize last year.
Highlighted below are the three Hearst Health Prize finalists and their award-winning projects.
1. Centering Healthcare Institute designed CenteringPregnancy, an innovative approach to prenatal care that has reached more than 125,000 pregnant women across the U.S. Studies have shown the care model helped reduce preterm birth between 33 percent and 47 percent and increased birth weights in preterm infants.
2. Community Care of North Carolina in Raleigh developed a transitional care management model that has reduced hospitalization and readmission rates for the target population by 10 percent and 16 percent, respectively, in the last eight years. The program also lowered total Medicaid costs by 9 percent.
3. Jersey City (N.J.) Medical Center created an incentive program to engage patients with complex chronic diseases, their families and caregivers in education, care management and healthy behaviors. Members enrolled in the program saw a 40 percent reduction in inpatient admissions and those with at least two chronic conditions saw a 32 percent reduction in cost.
