Southbridge, Mass.-based UMass Memorial Health-Harrington Hospital reduced the all-cause 30-day readmission for heart failure patients by 50% as part of a remote patient monitoring program with digital health company Brook Health.
Harrington provided patients with a scale and blood pressure cuff to track weight and blood pressure readings at home, according to an Oct. 8 news release from Worcester, Mass.-based UMass Memorial Health shared with Becker's.
Weight and blood pressure measurements were automatically transmitted to Harrington, where nurses could monitor the data and reach out to the patients when necessary.
The program reduced unnecessary hospitalizations, captured important patient health data, and improved communication and care coordination for patients and clinicians, the release said.
A recent study found remote patient monitoring and pharmacist support improved blood pressure in 74% of patients with treatment-resistant blood pressure.
