Wilmington, Del.-based Christiana Care's Department of Family and Community Medicine has been recognized by the National Committee for Quality Assurance as a patient-centered medical home, according to a health system news release.
The department's Family Medicine Center, which has two locations, is the first practice in Delaware to achieve designation as a patient-centered medical home. The NCQA's program is aligned with the joint principles of the Patient-Centered Medical Home established with the American College of Physicians, the American Academy of Family Physicians, the American Academy of Pediatrics and the American Osteopathic Association.
Key components include written standards for patient access and enhanced communications, appropriate use of charting tools, responsive care management with an emphasis on preventive care, use of health IT and evidence-based guidelines and measurement and reporting of clinical and service performance.
