UnitedHealthcare and Coastal Medical, a Providence, R.I.-based primary care group and medical management company with more than 100 providers, plan to launch an accountable care organization.
Through the ACO, Coastal Medical's providers will receive monthly updates on their patients in order to better monitor their conditions and maintain clinical information in an electronic medical record. Additionally, the providers can receive payment incentives based on improvements in measures such as readmission rates, disease management and prevention and patient safety, among others.
The ACO will be available to all Rhode Island residents who are part of UnitedHealthcare's employer-sponsored health plans. No start date for the ACO was released.
Coastal Medical is also part of the Medicare Shared Savings Program.
