El Campo (Texas) Memorial Hospital and its primary care practice, Mid Coast Medical Clinic, have joined the National Rural Accountable Care Consortium to provide more coordinated care and participate in the Medicare Shared Savings Program, according to the Victoria Advocate.
The National Rural Accountable Care Consortium helps critical access hospitals, rural health clinics, federally qualified health centers and independent physician practices in rural areas to combine and form ACOs. It helps these facilities meet the 5,000 minimum beneficiary requirement to participate in the MSSP.
As part of the consortium, El Campo Memorial and Mid Coast Medical will now provide patients with a care coordinator to help them navigate the healthcare system and avoid redundant care and tests, according to the report. The program promotes population health and wellness, and additional services will be provided for the most vulnerable patient populations.
