Roper St. Francis Physician Partners, a 220-physician network based in Charleston, S.C. is creating a patient-centered medical home in South Carolina to help improve health outcomes for BlueCross BlueShield of South Carolina and BlueChoice HealthPlan of South Carolina members who have conditions such as diabetes, hypertension and/or heart failure.
The initiative, which is supported by Columbia, S.C.-based BlueCross as well as Blue Cross and Blue Shield Association licensee BlueChoice, is aimed at providing preventive care to these high-risk patients to avoid acute episodes.
The overall goal of the initiative is to achieve healthcare's triple aim.
Nearly 25 Roper St. Francis Physicians Partners practices in the South Carolina communities of Charleston, Mount Pleasant, Goose Creek, North Charleston, Moncks Corner and Summerville are participating, according to a news release.
