Physician Direct ACO partners with Rite Aid to provide integrated care

Rite Aid announced its newest partner in the Rite Aid Health Alliance: Sylvan Lake, Mich.-based Physician Direct Accountable Care Organization.

The Rite Aid Health Alliance integrated care program takes a three-pronged approach to improving the care and health of patients with chronic conditions such as congestive heart failure, chronic obstructive pulmonary disease, high cholesterol and diabetes. In the program, physicians, pharmacists and special care coaches work together to provide comprehensive care.

Coordinating with the physician and pharmacist, trained care coaches work in-store with patients to devise personal healthcare plans for the patient. The plans may include medication management and meal and exercise planning.

The partnership will allow Physician Direct ACO physicians to recommend their patients with chronic conditions to the program, where patients will have access to on-going coaching and resources to improve health and self-management. As a Medicare shared savings ACO, Physician Direct ACO physicians focus on preventive care while working to improve the quality of care for and drive down healthcare costs.
