National Poll: Older Patients Want Coordinated Care From a Team of Providers

The John A. Hartford Foundation has released the results of a national survey finding team-based care, a key service of patient-centered medical homes, are preferred by older adults.

The survey made the following findings:

  • A vast majority (83 percent) of those surveyed who are already receiving well-coordinated care from a team of providers said team care has improved their health.
  • 61 percent of those surveyed who are not currently receiving well-coordinated team care said their health would improve if they began receiving team care.
  • 73 percent of those surveyed said they want well-coordinated team care.

The survey included responses from more than 1,000 adults aged 65 and older.
