Humana and Rockford (Ill.) Health announced Wednesday an accountable care agreement that went into effect May 1.
The new agreement includes value-based incentives tied to performance improvement and supports population health management with a range of tools, including predictive analytics, chronic care, disease management and wellness programs.
"We have a strong culture focused on quality, patient safety and continual improvement," Rockford Health System President and CEO Gary E. Kaatz said in a statement. "Creating this partnership just makes sense. Patients will be assured they are receiving the highest quality care following best practices. This will ultimately lead to better outcomes."
