Louisville, Ky.-based Humana and Cambridge, Mass.-based Iora Health announced plans Tuesday to expand their accountable care partnership with the launch of seven new primary care practices in Arizona, Colorado and Washington.
The organizations launched an accountable care and population health arrangement last fall that included the opening of two primary care practices in Phoenix and Seattle.
The new primary care practices, located in Phoenix, Tucson, Ariz., Seattle and Denver, are set to see patients as soon as August 1. The program is designed to provide Medicare Advantage members with access to affordable, quality care. The care is provided with integrated care teams of physicians, nurses, health coaches, medication management, behavioral health and social services resources.
Under the agreement, incentives are value-based and tied to performance improvement. The incentive arrangement focuses on quality outcomes as defined by the National Committee for Quality Assurance Healthcare Effectiveness Data and Information Set.
