Brookfield-based Integrated Health Network of Wisconsin has agreed to partner with Louisville, Ky.-based payer, Humana, on a three-year accountable care agreement for Humana's Medicare Advantage beneficiaries.
Under the agreement, Humana will apply its Accountable Care Continuum to IHN's 42 hospitals and 500 clinics to help transition facilities into value-based care models. The continuum is a stepped approach that allows facilities to take on various partnerships based on current infrastructure and reimbursement capabilities.
The agreement focuses on care coordination and personalized care and the organizations plan to implement predictive analytics, chronic disease management and wellness programs.
