Humana and HealthCare Partners Nevada, a Las Vegas-based division of DaVita HealthCare Partners, are furthering their accountable care partnership launched last fall by opening two new primary care practices.
The two new HealthCare Partners Nevada practices are designed to cater to Humana's Medicare Advantage members. The practices will use HealthCare Partners' Total Care Model, through which care coordination teams take a preventive approach to managing patient care and helping patients maximize their health insurance benefits. Humana will provide actionable patient data to help physicians better coordinate care and see total population health.
The additional two clinics, slated to open in time for open enrollment this year, will bring HealthCare Partners Nevada's network of primary care providers to 310 physicians, and its network of specialists to more than 1,700.
