Humana and Doylestown (Pa.) Health Partners announced Wednesday plans to offer Medicare Advantage members accountable care.
Humana will provide population health capabilities such as chronic disease management, predictive analytics and wellness programs to help Doylestown Health provide more integrated, coordinated healthcare for 3,200 Medicare Advantage patients in Bucks and Montgomery counties in Pennsylvania.
"Doylestown Health Partners is an ideal partner for this kind of effort because they've already made impressive strides in putting the patient and their primary care physician at the center of the healthcare equation," Rich Vollmer, Humana's president of senior products in Pennsylvania, explained in a statement. "That's the key to a better healthcare model. Our goal is to add our data and analytics tools to the mix, helping Doylestown Health Partners' physicians spend more time with their patients and better coordinate their care."
Humana now has roughly 1.5 million Medicare Advantage members cared for in more than 900 accountable care contracts around the country.
