Detroit-based ConcertoHealth has teamed up with Humana to launch a value-based care program for Medicare Advantage patients in Detroit and Southeast Michigan.
The value-based care program is similar to an accountable care organization in that it aims to provide more coordinated, personalized care for patients by tying value-based incentives to performance improvement and quality outcomes.
"We're very excited about our partnership with Humana across the greater Detroit area, and we look forward to introducing their members to our comprehensive model of care. This statewide partnership also offers us the opportunity to support Humana's patients in neighboring Michigan markets over the coming months and years," Dave Tigue, president of Concerto of Michigan, said in a statement. "This partnership demonstrates our mutual commitment to value-based care and to providing a more complete medical home solution to patients in the state of Michigan."
ConcertoHealth has a highly integrated healthcare network and several patient-centered medical homes. Under the agreement, these services will be complemented with Humana's population health capabilities to help further improve care for Humana Medicare Advantage members.
