To help support the health and wellness of its senior population beyond the clinical setting, Englewood, Colo.-based Centura Health's Colorado Accountable Care is leveraging the CaféWell Health Optimization Platform, developed by Welltok.
"As part of our efforts to transform healthcare delivery, we're focusing on preventive care and keeping our seniors, many of which are baby boomers, healthier longer," said Renae Pemberton, director of Colorado Accountable Care, in a news release.
The platform helps Centura's ACO and others with population health management by organizing an ecosystem of health resources. For example, a consumer using the platform who has diabetes will have access to an abundance of information on diabetes and will be able to confidentially interact with a diabetes coach who can offer guidance.
The platform also has a social aspect that allows consumers to interact with others, including those with similar health conditions. "A sense of belonging and peer-to-peer interaction helps patients," says Scott Rotermund, co-founder and Chief Growth Officer at Welltok.
Through providing personalized guidance, the platform is aimed at encouraging seniors to get more involved with their health. The platform provides resources for a number of chronic conditions; however, there are features that appeal to healthy consumers as well.
CaféWell allows users to set goals that can be as simple as being more active or losing weight. "When thinking of health and wellness, we're looking at the total population," says Mr. Rotermund. "The key is to optimize each person's health."
The platform is available online and on mobile devices, which some may think would present a challenge for seniors. However, most seniors want to stay connected, and they are using technology to do so, with at least 75 percent of Medicare recipients accessing the Internet at least once a day, according to an Accenture survey.
