Horizon BCBSNJ, Meridian Health plan ACO for OMNIA members

Horizon Blue Cross Blue Shield of New Jersey and Neptune, N.J.-based Meridian Health are collaborating to launch an accountable care organization for OMNIA health plan members.

Under the agreement, OMNIA health plan members who receive care from Meridian Health Partners, the system's clinically integrated network, will save on out-of-pocket costs. The ACO providers are considered "Tier 1" for the health plan, which means patients will have lower out-of-pocket costs and co-payments. The plan puts providers into tiers based on how much it will cost patients out of pocket to receive care there.

Meridian will receive payments from Horizon for coordinating the care of its OMNIA members. The physicians will have an opportunity to share in savings with Horizon if they meet benchmarks in clinical quality, patient satisfaction and cost savings.

OMNIA health plan coverage will begin Jan. 1.
