Heritage Provider Network, Fresenius Medical Care to build coordinated care networks

Marina Del Rey, Calif.-based Heritage Provider Network and Waltham, Mass.-based Fresenius Medical Care, a vertically integrated renal company, announced Thursday an agreement to collaborate and develop coordinated care networks and population health management services throughout the country.

Under the agreement, Heritage Provider Network will acquire or build coordinated care networks and Fresenius will bring chronic disease management services to those networks. The partnership looks to expand value-based care partnerships and drive clinical innovation.

"Our overarching objective is to improve the quality of life and clinical outcomes for people with chronic conditions such as end stage renal disease, by assembling a network of specialized services, including renal, vascular and hospitalist services," said Ron Kuerbitz, CEO of Fresenius Medical Care North America, in a statement.
