Health Net of California, a division of Woodland Hills, Calif.-based payer Health Net, announced it is launching an accountable care organization with Walnut Creek, Calif.-based John Muir Health.
The ACO will serve Health Net beneficiaries in John Muir Health's medical centers and provider network in three San Francisco Bay Area counties: Contra Costa, Solano and Alameda. John Muir Health is a nonprofit healthcare system that includes a network of more than 950 physicians.
Through the partnership, the organizations aim to lower costs, maintain access to quality care and improve health outcomes. The ACO plans to use care management and wellness and prevention programs to increase patient engagement and satisfaction. Programs include a patient-centered medical home and care coaching.
