Empire BlueCross BlueShield and New York City-based Mount Sinai Health System announced the launch of an accountable care agreement, effective Jan. 1.
The accountable care contract will cover Empire's 48,000 commercial and Medicare beneficiaries who receive care from Mount Sinai Health System, which includes more than 6,600 physicians and seven hospitals. The organizations are aiming to prevent duplicative services as well as preventable emergency room visits and hospital admissions, to drive down costs and improve health outcomes.
This will be achieved through more coordinated care, close patient monitoring and follow-up care, according to the report. Mount Sinai will be responsible for coordinating and providing care, while Empire will provide compensation based on cost reductions and measurable quality achievements.
Both organizations will track data to identify opportunities to improve costs and care.
