Community Healthcare System to form collaborative care relationship with Cigna

Munster, Ind.-based Community Healthcare System and Bloomfield, Conn.-based Cigna entered into a collaborative care relationship to improve access to healthcare, enhance care coordination and lower costs for residents of northwest Indiana covered by Cigna.

The collaborative agreement aims to close gaps in care, such as missed health screenings or prescription refills, reinforce the use of primary care physicians for preventative care and improve follow-up care.

According to the arrangement, registered nurse care coordinators employed by the health system will help patients navigate the health system and Cigna will compensate the system for those services. Cigna will reimburse CHS based on targets for lowering costs and improving care.
