Cigna, WakeMed Key Community Care Partners launch collaborative care initiative

Cigna is partnering with an already-existing accountable care organization, Raleigh, N.C.-based WakeMed Key Community Care, to enhance healthcare access, care coordination and affordability for Cigna beneficiaries in the Triangle area of North Carolina.

The collaborative care agreement launched Jan. 1 and covers roughly 20,558 members who will receive care from 370 WKCC physicians. These physicians will monitor and coordinate all aspects of patient care, with the help of WKCC registered nurse clinical care coordinators who will help patients with complex conditions navigate the healthcare system.

Under the agreement, Cigna will help support care coordination by providing patient-specific data to help pinpoint patients who might be at risk for readmission to the hospital, overdue for screenings or need a prescription refill.

WKCC will be compensated under a value-based financial structure for the medical care and care coordination services it provides patients.

This agreement marks Cigna's seventh collaborative care program in North Carolina.
