Cigna and Western Washington-based Providence-Swedish Health Alliance ACO announced Wednesday a new collaborative care initiative to improve care coordination, health outcomes, affordability, patient experience and access to care for more than 7,100 Cigna members.
The initiative will be one of Cigna's 124 Collaborative Care programs, which are similar to accountable care organizations. Under the program, patients will continue to see their Providence-Swedish Health Alliance providers, but receive monitored and coordinated care. The program uses registered nurse clinical care coordinators to help manage the care of patients with chronic conditions or other health challenges. Patients will also have access to clinical support programs from Cigna, including lifestyle management programs, tobacco cessation programs, weight management programs and disease management programs.
Providence-Swedish Health Alliance's 1,500 providers have the opportunity to earn value-based incentives if they meet quality and cost benchmarks.
