Cigna and PrimeHealth Physicians — a 45-physician independent medical group based in Miami — announced Monday the launch July 1 of a collaborative care initiative.
The initiative is one of Cigna's Collaborative Care programs, which is similar to an accountable care organization. This initiative, which will provide more than 5,300 Cigna beneficiaries with improved care access, care coordination, health outcomes, affordability and patient experience, will be Cigna's eighth in Florida and fourth in South Florida.
The program enlists PrimeHealth Physicians, PrimeHealth nurse clinical care coordinators and Cigna case managers to coordinate patients' medical care and help patients navigate the healthcare system. Patients with chronic conditions are expected to see the most immediate benefits of the program.
Under the program, physicians have the opportunity to earn incentives based on quality and cost benchmarks.
