Cigna and Harrisburg, Pa.-based PinnacleHealth are rolling out a new collaborative care initiative in central Pennsylvania.
The initiative, similar to an accountable care organization, aims to improve access to care, care coordination and improve health, affordability and the patient experience. As part of the agreement, physicians will monitor and coordinate patient care with the opportunity to be rewarded with "pay-for-value" incentives for meeting quality and cost targets.
The program also includes registered nurse clinical care coordinators from PinnacleHealth who will work with patients to navigate the healthcare system. The care coordinators will also serve as liaisons with Cigna's case managers, who provide patient-specific data to help identify patients in need of screenings, prescription refills or other medical and preventive services.
Cigna now has 134 collaborative care agreements with large physician groups across 29 states.
