The new collaborative care agreement will benefit nearly 5,000 individuals covered by a Cigna health plan who receive care from approximately 300 Holy Cross primary care physicians and specialists in south
Under the new program, Holy Cross physicians will monitor and coordinate all aspects of participating individuals' medical care. Registered nurse clinical care coordinators employed by Holy Cross will help patients with chronic conditions navigate the healthcare system, according to the news release.
Cigna will compensate Holy Cross for the medical and care coordination services provided. Additionally, the physician group may be rewarded through a "pay for value" structure if it meets targets for improving quality and lowering medical costs.
