Cigna and Decatur, Ga.-based DeKalb Physician Hospital Organization launched a collaborative care initiative Oct. 1 to enhance access to care, care coordination, health outcomes, affordability and patient experience.
The initiative, similar to an accountable care organization, is the fourth Cigna program of its kind in Georgia.
Under the program, 120 DeKalb PHO primary care physicians are responsible for coordinating and monitoring the care of roughly 7,100 Cigna beneficiaries. The physicians will be aided by nurse clinical care coordinators from DeKalb PHO who will help patients navigate the care system and work with Cigna's team of case members to align physicians with patient-specific data from Cigna.
The program uses a "pay for value" structure to award quality and cost achievements and Cigna will compensate the PHO for the services it provides.
