CaroMont Medical Group, a Gastonia, N.C.-based network of 44 physician practices in two states, has partnered with Cigna for a collaborative accountable care initiative, Cigna's version of accountable care organizations, effective today.
This CAC initiative reaches more than 3,000 Cigna patients who receive care from CaroMont physicians.
Under the program, CaroMont Medical Group providers will coordinate patient care, and the group's registered nurses will serve as clinical care coordinators and help patients with chronic conditions navigate the system.
The medical group can receive rewards through a pay-for-value reimbursement structure if providers meet quality and cost targets.
