Beacon Health Partners, a Medicare Shared Savings Program accountable care organization and independent physician association in Manhasset, N.Y., has signed a patient-centered primary care agreement with Empire BlueCross BlueShield, according to a News Medical report.
Empire's Patient Centered Primary Care Program is a value-based model, and Beacon will be rewarded financially for improving the health of patients in the program. Empire will also compensate care providers for non-visit-based clinical interventions like care planning, according to the report.
"We are convinced that our relationship with New York's leading insurance company will not only benefit the payer and all our member physicians, but will also pave the way for better, safer and more affordable care for our patients," Simon Prince, MD, Beacon Health Partners' president and CEO, said in the report.
