Baroma ACO to Collaborate With Walgreens

Baroma, a healthcare management company, has announced a strategic relationship with Walgreens to bring pharmacists and hospital staff together as part of Baroma's Miami-based Medicare accountable care organization, Baroma Health Partners.

The collaboration is expected to help reduce readmissions by improving medication adherence. Baroma implemented Walgreens WellTransitions program, a care transition program that provides continuous follow-up and patient support during discharge. Walgreens' pharmacy team will work with the ACO's care navigators as part of the collaboration.

"The health information landscape is fragmented, with individual groups or facilities often knowing only what they have prescribed or what their patients have discussed with them," Marisela Rodriguez, COO of Baroma, said in the news release. "Now, when a patient is discharged, we can inform our physicians of their patient's medication history…This access to information will help reduce hospital admission[s] caused by medication mismanagement or nonadherence."
