Calumet Medical Center (Chilton, Wis.). Calumet Medical Center is part of Appleton, Wis.-based Affinity Health System. The only hospital in Calumet County, Calumet Medical Center has served not only the people of its home county but also of three surrounding counties since 1956. Its campus not only includes a hospital but also a daycare and preschool. In addition to being a 2015 Top 100 Critical Access Hospital according to iVantage Health Analytics, the hospital also earned a five-star rating on CMS' Hospital Compare, based on patient responses to their HCAHPS surveys.
