Park Nicollet Health Services (St. Louis Park, Minn.). Park Nicollet Health Services participated in the Pioneer ACO program from 2012 to 2015. CMS selected Park Nicollet to become a Next Generation ACO in 2016. The ACO is part of Bloomington, Minn.-based HealthPartners.
