Baroma Health Partners (Miami). Baroma Accountable Care, known as Baroma Health Partners, joined the Medicare Shared Savings Program in 2013 and transitioned to a Next Generation ACO in 2016. The organization is designed as a full coordination model including health IT, health information exchange connectivity and individualized care plans for beneficiaries. Baroma Health Partners distributes 75 percent of the shared savings to participants and reinvests at least 5 percent into ACO infrastructure.
