NQF's National Priorities Partnership Illustrates 5 Health Improvement Case Studies

The National Priorities Partnership, convened by the National Quality Forum, has released five case studies detailing application and alignment with NPP's national priorities for improving healthcare.

NPP's priorities are modeled after the federal government's National Quality Strategy. The strategy's aims and goals include safer care, improved communication and coordination of care, promotion of effective prevent and treatment of chronic illness and more.

 



The following five case studies outline the various ways in which healthcare organizations align their goals with the National Quality Strategy:

1. HealthPartners. The case study focuses on how the organization leveraged NPP championing of population health and reducing racial and ethnic disparities.

2. Kaiser Permanente Care Management Institute. The case study focuses on care coordination and transitions, such as when patients move from hospital to home or into palliative care.

3. The North Carolina Quality Center for Hospital Quality and Patient Safety. The case study focuses on the center's adoption of national priorities to serve as a framework and to galvanize improvements in patient safety at hospitals state-wide.

4. The Pharmacy Quality Alliance. The case study focuses on PQA's partnership with NPP to drive population health improvement through better medication management.

5. The Spine Center at Dartmouth-Hitchcock Medical Center. The case study focuses on how the center leveraged NPP's heightened attention to reduce the overuse of care and improve patient and family engagement in care.
