Survey results from AHRQ’s Hospital Survey on Patient Safety Culture 2011 User Comparative Database Report show that less than half of respondents (44 percent) feel a nonpunitive response to medical errors.
The survey includes responses from 1,032 hospitals and 472,397 hospital staff respondents. Other key findings from the patient safety culture survey include the following:
• Forty-five percent of respondents reported safe handoffs and transitions of important patient care information across hospital units and during shift changes.
• Only 35 percent of respondents were not worried their medical errors were being kept in their personnel files.
• On average, most respondents within hospitals (54 percent) reported no events in their hospital over the past 12 months. It is likely that this represents underreporting of events and was identified as an area for improvement for most hospitals.
