Arkansas nursing home has licenses permanently revoked for patient safety issues

Just two weeks after the Arkansas officials recommended CMS revoke the care licenses for Happy Valley Nursing and Rehabilitation Center — CMS has done just that. 

The Arkansas Department of Human Services' Office of Long Term Care made the recommendation about the Malvern, Ark., nursing home in an Aug. 2 letter.

The 83-bed facility has "a history of serious quality issues but has not been formally flagged by the government," according to ProPublica.

In its formal letter, CMS stated that it found Happy Valley "failed to attain substantial compliance" with standards including the exercise of resident rights, keeping residents free from abuse and neglect, keeping the environment free from accidents and hazards as well as keeping them free from significant medical errors. 

The facility was slated to stop receiving Medicare payments as of Aug. 16 and full termination of the facility's agreement with CMS went into effect Aug. 19.
