Granite Healthcare Network chooses athenahealth to manage population health

Concord, N.H.-based Granite Healthcare Network has chosen Watertown, Mass.-based athenahealth's cloud-based population health management program to help manage and coordinate care across the network.

With patient insight provided by athenahealth's population health program, GHN hopes to be able to improve care quality and coordinate their growing network of risk-based payment arrangements.

Executive Director of GHN Rachel Rowe expressed optimism regarding athenahealth's program being able to help the network reach its vision of being the premier value-based provider in New Hampshire.

"athenaCoordinator Enterprise's insight into clinical, financial, and operational performance will enable GHN teams to better understand patient activity, identify gaps in care, and proactively coordinate care across our network," said Ms. Rowe. "With this intelligence, we can effectively manage our populations and influence patient activity to meet clinical quality goals, decrease hospital readmissions, and boost revenue and better health outcomes."

GHN is comprised of five independent hospitals in New Hampshire including Catholic Medical Center in Manchester, Concord Hospital, LRGHealthcare in Laconia, Southern New Hampshire Health System in Nashua and Wentworth-Douglass Hospital in Dover. Together, the network covers more than 80,000 lives.
