Walgreens has launched a care transitions and medication management program intended to help hospitals reduce preventable readmissions.
The company's WellTransitions program offers medication review, bedside medication delivery, patient counseling, follow-up calls and 24/7 pharmacist support for patients at risk of readmissions.
Hospitals will pay a fee for the service, and may select from two payment models: a flat, per-patient fee and a shared savings model. Under the shared savings model, Walgreens works collaboratively with the hospital or health system to set targeted goals for reducing readmissions, and savings are shared between the two groups.
The program has already been implemented at Washington Adventist Hospital in Takoma Park, Md., Sarasota (Fla.) Memorial Health Care System and Marion (Ind.) General Hospital. Walgreens plans to soon roll out the program across the country, beginning with the more than 150 hospitals where it operates outpatient pharmacies.
